13U/15U birth certificates

Aloha Thunder parents. I apologize for the late notice, but PAA 13U (and 15U) require copies of birth certificates. I have copies of some BC's but not everyone.

If you are on this list, we need a copy before the first PAA 13U game tomorrow (Sunday):

Kapaa Nishimura

Joshua de Jesus

Javan Montiho

Felcris Ahadain

Josh Roberts

If you can, please make 5 or more copies so we are stocked with them for future leagues. 

If you don't have a birth certificate, please bring a copy of a passport if possible. 

Also, for all 13U (and 15U) players, please print out the attached waiver form and fill it out before the game. Check your e-mail for the form. If you are not on the e-mail list, call or text me (551-1054) with your e-mail address.

Note: 15U doesn't play until Feb. 19.
